For use when the client is under a brokerage representation agreement

1. Fill in your information and your brokerage’s name.
2. Fill in the property address.
3. Fill in the consumer’s information.
4. Click the button “Share the form”.

The consumer will receive a unique link to a customized version of the form. You will receive an email confirmation once it has been shared.

The consumer can review it and choose to complete the acknowledgement section. If acknowledged, you and the consumer will receive an email confirming the acknowledgement.
